Improving Health Care Quality and Patient Safety

Stratis Health is an independent nonprofit organization that leads collaboration and innovation in health care quality and patient safety. For over 40 years, we've worked across settings of care to improve the health of individuals and communities, and to improve the processes used to deliver care.

Stratis Health facilitates learning and action networks across communities, and provides direct technical assistance to health care providers. We have four key program areas: health disparities, health IT, rural health and serving as the Medicare Quality Improvement Organization (QIO).

Examples of our program areas in action:

  • Working with 23 rural communities in Minnesota to start or strengthen palliative care programs.
  • Providing consultation and technical assistance to 23 clinics and 3 public health agencies to measure and enhance the cultural competence of their clinical and non-clinical staff working with multicultural patient/client populations.
  • Serving as the Regional Extension Center for MN and ND, we fostered success in adopting and implementing electronic health records with nearly 5,100 clinicians at 662 clinic locations and 121 critical access/rural hospitals.

Stratis Health works with providers in the field to develop and implement improvement projects, using measurement and evidence-based best practices, and translating research into practice, in order to facilitate sustainable improvement in health and health care delivery.

Medicare Quality Innovation Network - Quality Improvement Organization

Stratis Health works locally to achieve national health care quality goals, through Lake Superior Quality Innovation Network – the Medicare Quality Innovation Network - Quality Improvement Organization (QIN-QIO) which serves Minnesota, Michigan, and Wisconsin. We bring communities together for learning and action, support providers and practitioners with evidence-based clinical interventions and objective expertise, and improve the value of health care. We are:

  • Improving cardiac health and reducing cardiac health care disparities
  • Reducing disparities in diabetes care
  • Improving prevention coordination through health information technology
  • Reducing health care associated infections in hospitals
  • Reducing health care acquired conditions in nursing homes
  • Improving the coordination of care between health care settings
  • Improving quality through performance-based incentives and reporting programs

Stratis Health has supported quality improvement on behalf of Medicare for the state of Minnesota since the inception of the QIO program in 1971. We are one of 14 QIN-QIOs across the country working to implement national health care quality priorities on the local level.

Stratis Health Announces 2017 Building Healthier Communities Award Recipients

This Stratis Health grant award program supports initiatives that promote a culture of health care quality and patient safety in Minnesota. Awards ranged from $5,000 to $15,000.

Stratis Health Chosen to Provide Support for Small Practices in CMS Quality Payment Program Stratis Health will provide technical assistance in Minnesota, bringing direct support to thousands of Merit-based Incentive Payment System (MIPS) eligible clinicians in small practices.

Stratis Health Elects David Satin, MD, to Board of Directors David Satin, MD, is a family physician and pay-for-performance incentive program researcher. Dr. Satin will begin his three-year term January 2017.

Lisa Gall Selected to Serve on CMS Technical Expert Panel for the MIPS Quality Measure Development Plan The plan serves as a strategic framework for clinician quality measure development.

Indian Health Service Hospital Project Stratis Health through Lake Superior QIN is part of the HealthInsight team partnering with Indian Health Service hospitals to help them continuously improve quality of care for Medicare consumers.

Dr. Pederson to Serve on CMS Technical Expert Panel on IMPACT Act Quality Measures The panel will provide input on standardized cross-setting quality measures for use in post-acute care settings.

Minnesota e-Health Roadmap Released The roadmap provides recommendations and actions to support and accelerate adoption and use of e-health in behavioral health, local public health, long-term and post-acute care, and social services.

REACH - Regional Extension Center Program Concludes Program celebrates the success of clinics and small hospitals in Minnesota and North Dakota that have advanced their ability to use EHRs to improve patient care.

Palliative Care in Rural Minnesota: Findings from Stratis Health’s Minnesota Rural Palliative Care Initiative This Minnesota Medicine article describes a six-year initiative led by Stratis Health to advance palliative care in rural Minnesota.
